Our approach
Our coordinators work alongside participants, not around them. We aim to provide clear information, practical follow-up and respectful coordination that keeps the participant’s goals and preferences at the centre.
Referral information
We accept self-referrals, family referrals and professional referrals with the participant’s informed consent. Referral acceptance depends on capacity, location, suitability and the participant’s funding arrangements.
